If you have not already transitioned to Track 2, please contact us for assistance immediately, this is the last year that you can remain in the MDPCP program as a Track 1 practice. Please see our Services page which includes a link to our 2022 CTO Agreement. No need to sign the agreement yet, you will have an opportunity to do so once your practice has been paired with our CTO.
