A Message from Feroz A. Padder, MD, PHS Doctors CTO Founder and Managing Partner

Our vision of a care transformation organization is a simple one: we want to empower the physicians in participating practices to guide the direction of the CTO.

As the founder and the managing partner of Padder Health Services, a Track 2 practice in its second year with the MDPCP program, I am in frequent communication with our dedicated team of six Primary Care Physicians, two Physician Assistants, and a Nurse Practitioner. Although participation in the program has increased the demand on their time, they feel that it is truly worthwhile, as they are starting to see meaningful improvement in their ability to care for their patients.

Through a process of continuous improvement, we have devised a model that works extremely well in providing value-based care for our patients. At Padder Health Services, four Care Teams collaborate and provide feedback on best practices and recommend the resources needed for optimal patient care management.

We aim to replicate this model across multiple practices, not only to pool funds, but to draw upon the expertise of other physicians and providers in all areas related to patient care.

We thank the State of Maryland Department of Health and Medicare, for their vision of health care transformation through the MDPCP program, and hope to be able to contribute to its success as a CTO.

We encourage MDPCP practices who can bring a complementary set of experience and skills to our process to partner with us at PHS Doctors CTO. Together, we can have even a more meaningful impact!
